SDS Carer Information Session: Person’s Pathway

Self-Directed Support (SDS) Forth Valley offers friendly, independent advice on getting a support package through your local Health and Social Care Partnership. This summer, SDS Forth Valley will be running information sessions at Stirling Carers Centre every other Thursday. It’s a great chance to find out more about how SDS works, what the assessment involves, and the different support options you can choose from.

These sessions focus on the person you care for and explain:

• The process that would be followed if they need support with personal or social care
• The four SDS options for using an assessed budget to put support in place
